I saw a patient recently who was delivered by another physician, CLE, prolonged 2nd stage, VE assisted delivery. She had postpartum urinary retention, requiring a foley catheter until discharge. When the catheter was removed on the second postpartum she was sent home. 3d later she saw me for incontinence, she had a residual of >4000cc. It has now been 4wks, she is still doing intermittent self-catheterization, voiding 50-100cc, residuals of 150 to 400cc. What is her prognosis for normal voiding? Anyone have any experience with similar situations? When I discussed this with a urogynecologist, he felt she will never void normally, saying that overdistention to that degree will cause permanent damage to the small nerve endings in the bladder. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
