If she has such prolapse, she probably has a cystocele and rectocele as well. From your notes at this point I guess she does not have demonstrable SUI. Only if she does, would I perform a TVT pubourethral suspension, at time of vag hyst, and either McCall culdoplasty or sacrospinous suspension for vault support, with AP repair if indicated. Would be leary of gortex as a mesh as it has a higher rate of erosion in vaginal repairative surgery. J.Glenn Bradley MD
