At a conference I actually chaired last week, Devoe stated essentially the
same thing. A pre-forceps/vacuum note should be written that includes your
estimated fetal weight, the fetal position (head orientation and station)
and an outline of the consent. He stated as an example a recent case he had
"participated in" where if the documentation had been included, the outcome
of the case would have been different.
Personally, I do the same as y'all. I obtain verbal informed consent, do
the procedure and document thoroughly after...
