I would scope her again and if I found significant scarring or other pathology then I would do the salpingoohperectomy. If I found a normal ovary and tube then I would do nothing and tell her I don't know whats causing the pain.

If she was 42 y.o. and had completed her child bearing and understood the risk/benifits of being on ert I would remove a normal appearing ovary as I have done in the past, unfortunetly it only works 50% of the time at best .
