So this hematoma is within the myometrium then? If the patient wishes to keep her uterus, I think I would do a hysteroscopy, laparoscopy, and then a minilap in order to adequately evaluate her condition and excise the hematoma. It could very well be an area of adenomyosis with hemorrhage and these adenomas can be excised with the intention that future pregnancy will be attempted. Otherwise, do a laparotomy and a hysterectomy.
