At my institution the radiologist aren't great at gyn ultrasound and thus they recommend MRI routinely. The good thing about MRI is that it is not operator (ie tech) dependent, the bad thing is that the experience with gyn pathology is not very great. In the current issue of Ultrasound in Medicine their is a good reveiw of sonohysterography including some discussion of dislodging clots. I hada case of a rudimentary horn that with a hematometria that I decompressed during vaginal sonogram. This was missed on MRI.
