How is her count of white cells? and her pelvic USG? Is there any possibility of immunosupression? E. coli is quite capable to produce a salpyngitis, strong enough to cause an IPD (inflammatory pelvic disease). Tuberculosis could be another diagnosis. Fungus? This inflammation was accompanied by stinging, tingling, lympth node swelling, and urinary pressure. The cervix was friable and this inflammatory process progressed posteriorly involving the anus. Symptoms changed with some resolution of the vulvar sensitivity although she still has some irritation with the onset of suprapubic pressure which has been constant and has become more distressing the last two months. Night sweats, malaise and fatigue are also constant. She has noticed a correlation to change in bowel movements, pale stools, bloating with the onset of this discomfort and particularly two months ago when she presented with a colon infection, lasting five days. Temps have been up to 101 fahrenheit and she states that "exertion brings on the pain and fevers." Menstrual periods consist of three days of "thick mucus" bleeding followed by two days of clear fluid that is being expelled from the uterus. This clear liquid has recently increased in abundance and there is no particular odor associated with it. She states that the pelvic pressure is greatly reduced following her menstrual period. Past surgical history, myomectomy, 10 years ago, endometrial cryo ablation, six years ago. Multiple fiboids still exist. Numerous cultures taken showed a negative KOH prep. gonorrhea, chlamydia PCR was negative and vaginal cultures showed heavy E. coli. Doxycycline has offered relief from the pelvic pressure but has little effect on the inflammation. I will be grateful if someone can share their views with me. Sincerely, Olin Murphy Yahoo!
