Simply removing the device will clear the actinomyces. The choice is either take the device out and make alternative contraceptive arrangements, or leave it in and treat with antibiotics. If she is or becomes symptomatic it is better to remove it, but there is no need to waste all that penicillin on a 6 month course. Steve Richard Chudacoff, MD wrote:

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
