Bartholin's revisited

I know we've discussed this before ...

What would you do:

Mid 40's woman has a Bartholin's duct cyst which develops into an abcess and spontaneously drains.

1. hot soaks for 24-48 hours and just observe (how soon - 1 month, 6 months or something else)

2. go to OR and marsupialize and biopsy the duct wall (because of others recommending biopsy of any Bartholin's if the woman is over 40) (I have seen two patients with Adenocarcinoma of the Bartholin's gland - duct)

3. Something else (and describe)

If you biopsy - WHAT do you biopsy - the lower end of the duct where you are marsupializing - or further up???

Thanks - I look forward to the discussion!

Joanne

PS - she is the wife of an ObGyn (who quit medicine a few years ago) ...

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
