The ischialrectal fascia will collapse durring the healing so you end up with the classic Grand Canyon defect along the suture line, I have to remember when doing a routine gyn exam on these patients years latter not to say well it looks like you had a little episiotomy.

The last time I saw a medilateral epis. was about three years ago the patient got transfered from eleswhere because of postpartem siezures, the nursing staff called me because they where worried about the way the perinium was looking, so I go see the patient and then go tell the nursing staff not to worry it looks normal its just a mediolateral episiotomy.
