I think I have cut one mediolateral in the last 10 years, recently, as a matter of fact. I may cut 3-4 episiotomies a year, period, all midline. When I did cut the mediolateral, I cut proximal and lateral to the anal sphincter, rather than at a 45 degree angle, as I was taught by midwives. It seems that the repair was easier, less blood and less post-partum dyspareunia. However, my series is slightly less than 1000.

I can think of 2 in about 1800 births.

One was a woman from Ethiopia who had been genitally mutilated and infibulated. The other was a recent shoulder dystocia. She had a short perineum and visible scars of previous mediolateral episiotomies to both the right and left. When I was unable to extract the posterior arm I cut a right mediolateral along the old scar.
