I have done one mediolateral epis in approximately 1700 births. It was a VBAC in a woman w/ Crohns who had had major rectal involvement.

Unfortunately, there had been no assessment of her perineum relative to her Crohns prior to labor. As she was pushing, it was obvious that she had a perineum that was 1/4 inch long. I did a a hockey stick mediolateral epis. There was no way that I wanted a tear into that rectum.
