Between 1993 and 1997 I collected data on all the infertility patients presenting to this hospital in KwaZulu Natal (all Zulus). There were 462. Two hundred and four never came back for the second visit because they didn't want to have an HIV test! Only those partners who had no children were sent for semenalysis - there were 40. Sixteen refused to be tested, and of the 24 tested, 16 had an abnormal result. 41% of the women were anovulatory, but 84% of those who were ovulating had an abnormal HSG. I believe that we cannot dispense with HSG as, in contrast to laparoscopy, this shows the insides of the tubes, whereas the laparoscopic dye test shows the outsides. Of the 85 cases of tubal blockage only 23 were suitable for tubal surgery. Unfortunately, the majority of those who became pregnant after tubal surgery never came to tell me about it, so I cannot report the percentage success rate - only finding out by accident if someone doing the A/N or delivery happened to notice the history and tell me.
