A "system" error I'm sure (and you can ask her about that next time anyway - if you remember or the post-it note doesn't fall off the chart). But you've made the point that we are all human and it is quite natural to overlook such details when clinicians don't have some kind of a redundant, fail-safe system to make sure these kinds of things aren't missed. Patient care shouldn't depend on how much sleep we had the night before or that we were thinking about a hundred other things at once.

BTW, what percentage of Caucasian patients *should* be screened (or at least offered - in writing) for CF carrier status in your opinion?
