GEN: Antibiotic SelectionFrom: Garry E. Siegel, M.D. (garrys@mindspring.com)Sun Jun 29 14:49:52 2003
I am interested in hearing which antibiotics people use, and why, for IV treatment postop infections, such as: endometritis after section postop cuff cellulitis/fever after hysterectomies etc. In residency (completed in 1986), the standard was Clindamycin/Gentamycin, perhaps with Ampicillin/PCN. We also began to use a single agent, such as Cefoxitin, subsequently replaced by newer Cephalosporins such as Cefotetan. When the super penicillins came out (Timentin, Zosyn), these also became popular as single agents. Recently, I had a patient post TVH who went home on day 2, became febrile over the weekend, and, long story short, was readmitted on postop 5. Because of an allergy to PCN and Keflex, I used Gent and Clind, and she didn't really get better until she was changed to Invanz (ertapenem)per an infectious disease specialist, and responded. So, what do you guys use, and why? BTW, I'm not talking about prophylaxis, but treatment. Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
