I have a young woman (late 20's) - completed her child bearing - with an RV uterus and dyspareunia. This is completely reproducible by palpation of the fundus on pelvic exam. No uterosacral pain etc. I plan on doing a uterin suspension. As we do these infrequently - I have usually done a Gilliam (?) - pull a knuckle of round ligament through the rectus fascia and sew it to the fascia - bilaterally. The few others I have done on very selected pateints have worked quite well. I'd love to know what others are doing and whether there is a better - non laparotomy way I can incorporate into my armamentarium... Of note - she had no dyspareunia with her first partner - before marriage, and has always had this with her current partner - her husband. It is somewhat worse since having the 2 babies - and he definitely has a longer erect penis than her first partner. They considered measuring his erect length before she saw me in case that helped my evaluation.
