Me - Canada. My doc put me off work early Dec. 98 as the pain was soooooo bad, I was physically and emotionally a mess. Got one cheque from the insurance company that covered two weeks only; the letter cracks me up now, cause they wrote something inane like 'we would not expect problems from this condition to last more than two weeks...' ha ha ha!!! They wanted copies of reports, consult notes etc etc to be submitted, if I wished to continue with my request for payment - I have yet to receive anything more, did have surgery in Feb., and have been back to work 3 weeks.
