Re: SSI & Endo question (s?)From: Caroline (anonymous@obgyn.net)Wed Nov 18 12:24:33 1998
Jenny, I am currently on disability for endo. It did not used to be covered as a disability but I believe someone in the Endometriosis Association successfully lobbied for it to be classed as a disabling disease. The first step is to get a disability form. In CA this can only be done by having the disability office send the form to you, but some employers keep copies (ask you HR dept). You fill out your portion of the for and then take it to your Dr. Your Dr fills out the diagnosis and sends it off. They will then send you a check and another form for your Dr to fill out. Each check is accompanied by a form for your Dr to state you are still disabled and eligiable to continue receiving the benefits. The max benefit is based on your income level and is on the form (sorry I don't have it still around). Best regards, Caroline
