Hi Sue, I just had a lap on Friday and had endo excised in my cul-de-sac. I went to an endo specialist as well who removed it through traditional excision. What he explained to me was that the cul-de-sac like like a pocket and they lift the uterus to access it. They then will turn the pocket inside out and excise whatever is necessary. He showed me pictures of how it was done, so it made sense (but also looked pretty gross!) I honestly have not really felt that much pain in my abdomen, only slight cramping. The majority of my pain was from the gas left in me after the surgery and the incisions themselves. I have noticed that my abdominal muscles seemed to weaken since the surgery and am having a problem with producing a "satisfying" bowel movement, but I can't directly link that to removing endo from the cul-de-sac as I had a presacral neurectomy and appendectomy as well.
