Asking for your thoughtful advice

Hello ladies, I am looking for advice here, and I really appreciate any guidance you can offer. This is a bit long, but I thought it would be helpful to include my history.

I have been suffering with endo since the age of 15. I used to throw up for about 2-3 hours on day 1 of my period, with horrible cramps, fainting, and dizziness. I have since had many endometriomas on both ovaries, with one emergency surgery and partial oophrectomy [ovary removal] due to a ruptured endometrioma in 2000, and another surgery last year on the other ovary for more persistent endometriomas. My surgeon removed most of ovary, 6 endometriomas, and the tube - but some of the ovary was stuck to the bowel and my uterus, so he lazered any other endo and left this piece of ovary in place.

Of course, it is a year later, and I have another large endometrioma on what appears to be what is left of the right ovary. It is adhered to bowel and uterus. I have been to two different doctors. Also, I am no longer getting my period and my doc says I am not ovulating. My CA 125 level is only 23. One doctor is an excision specialist and this doc suggests we remove what needs to be removed and/or excised. Also, a general surgeon will be on hand with this doctor in case the bowel needs to be resected. This doctor is several hours away and affiliated with a good hospital in NY. However, this doctor is not board certified.

The other doctor recommends removal of both ovaries and uterus - due to repeated endometriomas. He says this way I can go on HRT after surgery. This doctor is near my home and part of a reputable hospital near my home. This doctor is board certified and has a large support staff that is easily accessible by phone or car [10 minutes away].

I am very confused about which way to turn and I would be most appreciative of any advice or guidance that you can offer. I am in my 40's and will not be having any more children - even though this makes me very sad. :(

I am extremely concerned about having too many surgeries as well as the risks of ovarian cancer, but I also have concerns about the recovery and after-effects of a hysterectomy.
