endometriosis/low back pain

Please, I need advice. First, endometriosis was confirmed with a laparoscopy at age 21. Scar tissue was removed as well as my appendix due to scar tissue. Since, I have had 2 kids and have not been on birth control. For the last year, I have been having low back pain, and it escalated in the last couple of months. One night at work we thought I hurt my back and eventually had an MRI done. Results showed a normal back and surrounding tissues, but did visualize a mult-cystic "focus" presumed to be my right ovary. I scheduled an appt. with an OB, but things have gotten worse in the last couple of weeks. The pain is getting worse, and now I'm having daily dizzy spells, elevated temperature and pretty much non-stop nausea. It's so severe now, with the dizziness that I haven't been able to work. I decided to go to Charity ER b/c something is just wrong. A vaginal ultrasound was done, but no bloodwork. The results showed 3 cysts on my right ovary, 2 major. However, I don't feel this explains all the symptoms. I do know that the back pain is probably from endometriosis, but I can't figure out the dizziness and nausea. Where do I go from here. The appt. with Charity OB is in 2 weeks, but I'm struggling making it through each day. I'm assuming that I need a laparoscopy but I'm not sure that the OB will agree. Should I find a new OB that will work with a payment plan, and how much of an emergency is it? Is cancer something that needs to be ruled out? Also, my last period was very abnormal. Usually I'm regular but heavy. This last time, the cramps were crippling and I only spotted. I never actually had a full menstruation.

To anyone that gives me time and consideration, thank you so much.
