I would suggest calling his office, and leaving a message with the nurse for him to consider the symptoms. It's very common for people to forget things when going to their doctors. Just tell the nurse what has been going on, that you were in not too long ago, and that you would like the doctor to put this in with his thoughts on how to begin treatment.

If you are concerned that it is endometriosis, then I would suggest talking to him about a lap that way you can know for sure if that is what it is and get the proper treatment to hopefully help you conceive.
