I would like to fill one of these out. I have fibromyalgia from taking Lupron. My Rhemoutoligist has confirmed that but will only confirm its myalgia from Lupron. I was wondering if you had answers to any of these questions? Will This effect me, my healthcare? or is it annoonoymous? Is there anything I should know before reporting? Which forms do I fill out, Im a bit confused about the whole thing. Thanks, Jessica
