For straight up pain management, you might try Dr. Claudio Palma at St. Luke's in SF. I've been seeing him for over a year, and he helped me a lot. He also knows how to taper someone down off the meds slowly enough to avoid nasty withdrawals, when you are ready for that.

If you are looking for an endo specialist, Dr. Cook in Los Gatos is among the best, and he does pain management as well. Only downside is he's not in any insurance networks, so you have to have a PPO to get it covered at all.
