Hi and sorry for delay in response Progestins are usually thought of as progesterone like substances. Progesterone is a specific molecule that is made within your body, but there are many other formulations with "progesterone like" activity that are synthetic. All birth control pills have a predominant progesterone activity although they usually contain both estrogen like (synthetic estrogen) and progesterone like (progestin) hormones. The birth control pills are generally thought to reduce the rapidity of progression of endometriosis rather than having much impact on existing implants. Surgery can reduce implants and medications like lupron or femara (that dramatically reduce estrogen production) or progestins (including progesterone itself or any of the synthetics, which act to reduce the bioactivity or effects of estrogen) have been shown to reduce implants. Birth control pills have not been shown to do this significantly, although they can help with the pain of endometriosis in many women (possibly by reducing the amount of growth of endometrium which is estrogen dependent, the amount of menstrual flow, and the flucuations in growth of the endometriosis implants). The progestins used for endometriosis treatment have varied. Provera has been studied in the past and research suggests a significant impact on implants and pain (though there can be lots of side effects and the return to ovulation may be delayed after stopping the medication). Norethindrone has also been studied and has been shown to have a significant impact on implants and pain... this is often used in add back treatment if lupron management is extended past 6 months.
