There is surgery with an excision specialist who can thoroughly remove the disease. They get the best results, but there aren't many doctors who can do it right. Here are some links:

Fatigue just seems to be part of it, for me anyway. Our bodies are battling something growing inside of them that isn't supposed to be there - it takes a lot of energy, even if the pain is controlled. (But pain makes it worse.)
