I had exactly the same experience. Having an internal ultrasound left me in agony for days. I didn't have hives but I did have severe cramps that incapacitated me. When the doctor did an internal I was wincing in pain and he suggested that I had Adenomyosis because of this issue and also because he saw it on ultrasound. I have had lots of pain issues with inserting tampons too, and wondered why this was. As soon as I put in a tampon I would double up in pain from cramping. When I had my lap a month ago, it was confirmed that I have severe Adenomyosis that has eaten two large holes in my uterus and now I need to have a hysterectomy. Unfortunately, while laps can help Endo, they cannot help Adeno as it can only be cured by a hysterectomy. It can be tentatively diagnosed by MRI or ultrasound if your doctor is very experienced, but it is best diagnosed by uterine biopsy, as mine was. Are you planning on having a lap? If so then make sure they do a biopsy.
