Dr.Daiter, I think you are a godsend to many of us-could you please advise again?From: anonymous (anonymous@obgyn.net)Sat Jun 2 20:54:57 2007
I will be quite honest this is very lengthly and may include some graphic info. Dr. Daiter, I started posting here about a month and a half ago and you have responded to a couple of my posts. I just had a laparoscopy on the 23rd and have my follow up on the 5th (Tuesday). I was hoping you could advise me as to what questions would be best suited at this appointment. I have many things going on, and my doctor has been compassionate in the past-however lately I think he has be overextended and may be thinking that I am a hypochondriac (sp?). Bless you if you can bear with me through this very lengthly list. I have had endo for 11 years I had a lap in 97, 2003, and 5/23/07. All 3 times endo was found the 2nd revealed stage 3, and I am not sure about this last one until I speak with my OBGYN. Before I started to mensturate at age 11 I NEVER had any type of cyst or strange lump. I now have 6 cysts on my scalp-3 worth removing (I am told they are nothing to worry about unless they hurt or are visable-2 are visable and 2 hurt (one is the size of a quarter around, and sticks up about 2 pencil earasers high). I was also diagnosed with IBS and fibrocystic breast disease which you will need to know further in this list. I guess to get one question out of the way-is it worth asking my doctor if these cysts on my scalp could be endo? I always thought unlikely, but now I am not sure. The last year and a half I have had 10 UTI's well some of those may not have gone away from the ones prior, but-from where I stand it is as many times I was told I had an infection. There is a ton of pressure when I go and there is urgency-I was actually urinating in my pants because I got the sudden urge and couldn't go 50ft to the bathroom. I also had lost bowel control approx 2 times per month-usually close to my period. My doc said it was urgency incontinence and put me on detrol. Since my lap I have not taken the Detrol because it could cause constipation, and I am having a hard time with that since the surgery and did not want to have to strain more than I have to. Interesting enough since the surgery the urgency is not as bad-I have not lost control once. In my prior posts you stated that I should see a urologist and confirm there is not a kidney stone. The OBGYN has also had extensive training in urological issues and was capable of handling this (according to his credentials). During the lap on the 23rd he also performed a cystoscopy-according to my mother (he spoke to me in the recovery room, but I do not recall exactly what he said-but he did talk to my mother). He did not find IC or any other issues other than scar tissue and signs of infection. He is going to be doing an ultrasound on another area -do you think it would be out of line for me to ask him to do an ultrasound of my kidney (to rule out stones) at this time? The right flank is the main source of pain when I get the infections, and actually even as I sit here tonight I have a dull ache under the ribcage-that comes around to the front a bit as well. Okay now a couple of things that were briefly discussed just before my lap that the doc said we would need to discuss further. During my period last month, my bleeding was more spotty. I had a BM and was straining a bit. When I was finished there was a golf ball sized (no exageration)-clot in the toilet with my feces-it was indeed a blood clot-and started to pull apart-filmy separation as I flushed. Later that evening my son was laying on my stomache, and across my chest (mind you I have not been breastfeeding for over 6 months). I felt like my shirt was wet around the nipple, so when I went to shower that evening-I put pressure on my right breast and indeed there was a yellowish/white (concentrated milk). Not like I got sprayed in the eyes or anything, but it was present. Now my breasts have been tender for about 6 weeks or so, and I have noduly breasts due to the fibrocystic breast disease. When I mentioned this to my obgyn he first addressed the clot asking where it came from the rectum or the vagina (to me I had pressure and pain from both-so I wasn't sure-my menstural bleeding increased to normal period amounts after the clot was passed, but I couldn't tell him for sure). Then he addressed the breast issue (which I thought was just a simple hormonal imbalance-temporary and would never happen again). No-he gave me a very serious face (like I should have called him sooner). He then said he would have to do an ultrasound of my breast-when I mentioned it being hormonally related he said he agrees that it is probably in part hormonally related-that he would do a test to see if my brain was producing to much prolactin (that is not good is it?) Okay I think this is my final issue-after my lap-about 6 days-I woke up at midnight and thought I had to go to the bathroom. I went downstairs and did go, but when I returned upstairs I got the feeling like I needed to go again. I went back downstairs and sat on the toilet-I couldn't, I strained and it hurt. Then the pain traveled up through my whole back and abdominal region-it was as if I had my very worst UTI it felt like all of my organs were trying to fall out through those openings. I went down and took a percocet (sp?) and 400mg of ibuprofen, the percocet was given at the time of surgery-I do not take them on a regular basis for pain and just to give you an idea of my pain threshold-I returned to work a day and a half after my scope without anything for pain (I did only work a half a day-but came home to do laundry and dishes and take care of my 2 small children). Okay, so back to this horid night. The percocet and ibuprofen did not work. I grabbed a large ice pack and layed directly on it and cryed for about 45 minutes before falling asleep (I considered going to the hospital, beings the pain was at about a 9 on the pain scale), unfortunately I had heard news that my father was already at the hospital with chest pain-so my parents could not help me with my children who were obviously asleep at this point. So anyhow I did fall asleep crying. I woke at 3:30 and again was balling because of the pain. My perscription said 1-2 pills every 4-6 hours so I figured I was safe in taking another, and I swapped out my ice pack for a fresh one (unfortunately I could not get back to sleep, and did have to work that day). I went to work-at this point the edge was taken off of the pain since I am not used to taking so many pain meds-I was more high than anything. My intention was to call the clinic at 8:00 to ask my doctor what I should do-if this could wait, or if I should see him or go to the emergency room (my kids were at school and day care at this point). Alright so I get a phone call at 10 after 8 (I hadn't had the chance to call my doc yet)-the call
