This is a tough situation, to be sure. You should definitely look into
pain management. It may take a while to find a doctor, so it would be a
good idea to have a pain doctor working with you. It's made a huge
difference for me.
Endo specialists are few and far between, and as far as I know the
handful of top endo docs aren't in any insurance networks, and none take
medicare nor medicaid.
What might work for you is to try a teaching hospital. You may be able
to find a good doctor who can perform excision at a teaching hospital
that takes medicaid. Whatever doctors you see, you should talk with
them and make sure they have a lot of experience excising endo. Here's

Honestly though, even with HMO insurance I haven't been able to find a
doctor in network with good endo skills - there just aren't very many of
them out there. It looks like I'm going to be borrowing the money for a
specialist from family, and I feel extremely fortunate that's available
to me.
The insurance/healthcare situation in this country stinks for a lot of
us! I'm sorry you are having such a hard time, and hope something works
out soon.
