I know it's a little late now but what I recommend is every time you have a visit copy the paperwork and keep them is a file folder. I have a copy of every doctor thing that I've been through for the past two years and if any one needs it I have it tucked away. just for future reference.(Horribly my active duty medical record for two years is thicker then my coworkers who have been in for ten.)
