my docotrs office called the company and they sent it to them. i was
lucky my insurance company paid for it. i did three shots and they
didnt work..had lots of side effects which i didnt care about but my
left overy that was infected before my lap and the doctor took it off
came back so i went off of it, but not untill the doc asked for a three
month supply, so now i'm waiting for them to take it back and give me my
copay back. just ask the doc to send a letter, and tell them the reason
why you need it. i hope it helps you! Mel
