I've seen this post routinely in the past couple of months on this site. The wording may not be the same, but the request is. If you want to do a report on women's health issues concerning Endometriosis, why not use the Internet to look up statistics of the facts you seek, and find other articles to site in your work? And did you know that you can cite archived forums as long as you give credit to the source that you found the information? As far as a questionaire goes, that is something more personal. Are you copyrighting the questionaires that you are having women with Endometriosis fill out? If not, then you cannot cite your survey as a source of information, and having these women give you their personal information serves unsuccessful for the data you are trying to build.
