I am not really sure if an endometrioma itself indicates endometriosis but I think it is a major possibility. When I had my first lap done, my doctor found an endometrioma on my left ovary, also known as a chocolate cyst. I also had endometriosis lesions all over my uterus, uterosacral ligaments, bladder peritinum, adhesion on sigmoid colon, and bowel areas. None of it, including the endometrioma, showed up on the ultrasound itself. I found this site where a woman tells her personal story about how she was diagnosed with endometriosis through a lap and had had an ultrasound first which showed an endometrioma.
