You might want to watch filling out that form due to previous conditions might be exempt fomr being paid for by your new company. I know some companies state that if you have not received treatment with in 6 months that if you get new insurance and try to make them pay for it they won't. I would ask your benefits administrator. I used to work for an insurance company and that is the worst job ever!! Rhonda
