These are questions that really should be answered by your doctor - any
other person who tries to answer them is only guessing and you're still
no further along to understanding than you are right now so, call your
doctor's office and ask them to help you understand the paperwork that
they have asked you to sign - that is why it is called 'informed
consent'.
