Having a hysterectomy only guarantees you will no longer have periods or be able to carry a pregnancy.

If one or both ovaries are left intact you will continue to have estrogen levels go through the regular rise and fall of your cycle, you will even ovulate but there will be nowhere for the ovum to go and it will be absorbed by the body.

If one or both ovaries are removed you will have decreased estrogen production (or none) from the ovaries. If one ovary remains you may not need HRT at all or a short course immediately after surgery to combat menopausal symptoms IF they prove to be highly bothersome. If both ovaries are removed you MAY CHOOSE to take HRT to combat menopausal symptoms IF they prove to be highly bothersome.

HRT is not an absolute must, it is something you choose to take and you can also choose to stop taking it. There are various formulations and methods of delivery to be considered as well.

Theory is that without the ovaries producing estrogen the endometriosis will have nothing to feed it and that will be the end of it. HRT provides estrogen which feeds endometriosis and it can indeed come back.

Hysterectomy is major surgery and life-altering. Be sure you discuss all the pros and cons, risks and benefits with your surgeon prior to consenting to surgery. Be sure you discuss HRT and make your wishes known.
