Geeze Beth, you've already gone through all the usual measures I'm not
sure what else you can try on your own. Has anyone suggested pain
mapping?
Dr. Carter has some really good articles on OBGYN.net that you can find

he's in California but he's supposed to be one of the top Gyne surgeons
in the country with special interest in pelvic pain.

It sounds to me like you have tried all the 'easy' stuff and you're in
need of more serious attention. Make sure you log your symptoms daily!
The one article from Dr. Carter has print outs with it that helped me a
lot.
Good luck and try, I know, easier said than done, but try to keep your
stress down because it will make the pain worse.. do whatever you can
to help destress and relax in the evenings so you can sleep.
