My first thought would be that there is some sort of polyp, laceration, fissure or other physical irritation present in your vagina that is being further irritated by intercourse which then causes the bleeding. I say this because you say it has only happened 3 times in the span of a year. Try to think back to those times and see if you can determine any sort of pattern. For instance, did you have any type of itching, burning, discharge, urinary tract infection within the day or two before or after the intercourse/bleeding incident? Do you use tampons? Is it possible that the insertion device has made small cuts? Do you use sex toys? Is the sex vigorous? Are you well lubricated? Has your doctor mentioned any physical traits regarding your genitals and reproductive organs, such as a retroverted uterus? It's certainly worth mentioning to your doctor and making an appointment to be seen as soon as possible after the next time it happens. That way, if there is a tear, cut, polyp or such it should be easy for the doctor to see and then come to a solution for you.
