Usually menstruation is a result of the disintegration of a corpus luteum cyst (the kind that is left behind after ovulation), which causes a drop in progesterone, which triggers the lining to shed. Sometimes the uterine lining just gets to a point where it can't sustain itself any longer and it sheds, with or without ovulation. Sometimes a hormone shift (for whatever reason) triggers the lining to shed. Get a copy of Toni Weschler's book, Taking Charge of Your Fertility. Explains everything reproductive in nature (at least "normal" reproductive stuff).
