I live in the Cincinnati area (in SE Indiana) and I have Humana insurance. They they pay 100% of my shots. I had a lap in June of this year and all it cost me was a $100 co-pay. Pretty good considering the surgery was almost $7000. I don't know what it costs per month though. It is insurance provided by my employer and only costs me $15/week. My employer is located in Northern Kentucky. Good luck in finding a better company. Nothing more frustrating than not being able to try different treatments because of the cost.
