  your doctor can fill out this paperwork which will states that you have a medical condition which requires you
to have periodic visits to the doctor, for an episodic incapicity rather than a continuing period of absince.
this will give you up to 12 weeks of unpaid leave that you can use with no danger of using your job.
  i have to take 2-3 days a month of because of incapacitating pain and my docotr was glad to fill the
