If it is related to movement or stiffness, my guess would be muscles. If this pain jumps from shoulder to shoulder, it is probably muscle, if it stays in the same shoulder, it can also be a sign of ovary or gallbladder pain depending on which shoulder, based on referred pain signals. Hope this helps.
