I had a endo removed and hysterectomy with both ovaries left in because they looked/seemed perfectly healthy. A few months after surgery, I was having horrible right side pain (left hurt but not as much). Ultrasound showed cysts coverning both ovaries (larger cysts on right hence more pain on that side). So just 10 months after hysterectomy, I was back in hospital having both ovaries removed. So to answer your questions, it might be adhesions, endo, or cysts. When they removed my ovaries, the endo was also present once again. As far as the surgery, it was similar to the hysterectomy. I had one incision in belly button and two side incisions (one on left side and one on right). I would call the doctor especially if the pain is starting to effect your everyday life.
