It's not the size of the endometriosis implants nor the amount of endo that causes the pain. It is the location of the endo. That is why women with stage one endo can experience debilitating pain. However, it is not true that the less endo there is the more it hurts. I know, I have stage four endo and am in constant debilitating pain, and I am on continuous birth control pills. One of the reasons that women still experience pain after their lap is because the doctor didn't remove all of the endo. Some of it can be very small, or it can be hidden under other tissue and therefore disguised. And some doctors don't even know that endo can present itself in a variety of colors, so they only remove what they recognize and leave the rest there.
