I have PCOS and in July 2002 had a hysterectomy at which time I wanted my ovaries removed due to multiple cyst problems. The doctor didn't remove my ovaries even though it took him almost an hour to get past my severe adhesions from 3 c-sections and appendicitis. Three weeks later I was an emergency readmit with ovarian cysts and ovarian torsion. The torsion cleared itself up overnight before further surgery. For the next few months I was on Lupron to shrink the cysts and hopefully stop them from ocurring again. In December 2002 I called my GYN and was transfered to the on-call physician and was sent to the ER with severe pelvic pain. The ER doctor (not a gyn) did a pelvic exam and bloodwork, then said: "it's a cyst" here's a shot of pain medication and follow up with your PCP tomorrow. I called my PCP the next morning and they scheduled an ultrasound (which should have been done in the ER the night before). The ultrasound showed a softball sized ovarian cyst on the left side and a smaller cyst on the right side. The next morning my pcp saw me and scheduled removal of my ovaries for 9 days later. I begged him to do it sooner because I was in severe distress, I couldnt' sit up and there was so much pressure on my bladder I lived in the bathroom. Finally, just 2 days after my office visit it got so bad they did the surgery the next morning. What they found was a football sized cyst on the left side and a grapefruit sized cyst on the right side. Both were heavily stuck to tremendous amount of bowel and abdominal wall. After 3-4 hours of surgery he couldn't remove all the ovarian tissue. The morning after my surgery he came to me looking grim (because for the past 5 months he had been saying that he should have taken the ovaries in the first surgery. He says this over and over again.) He explains to me about Remnant Ovarian Syndrome and puts me back on Lupron with all it's nasty side effects. FInally I had to go off the Lupron after 3 months because it was causing me severe dizzy spells, joint pain and body aches. During my 3 months on the Lupron (every 28 days) I had cysts but smaller. They all resolved themselves, but here I am, 2 surgeries later and still getting regular ovarian cysts. IT's very frustrating. The people at TAP Pharmaceuticals told me that Lupron wouldn't work on my because of my weight. I called them when my GYN wanted to move my shots to every 21 days. They said if you are extremely obese, which isn't uncommon for PCOS sufferers that the Lupron will shut down estrogen production in the ovarian tissue, but will not stop fat cells from producing estrogen. After weeks of trying to get a call back from my GYN I gave up and am in the middle of filing a lawsuit against him because he should have taken my ovaries out during the first surgery. If it takes well over an hour to find the uterus and ovaries during a hysterectomy due to severe adhesions and your patient has a documented history of PCOS and ovarian cysts, shouldn't the ovaries have come out at the time of the original surgery (at which time they were not adhered to anything, per the operative report for surgery 1). I'm really frustrated with this because I'm still in pain, I still get cysts and during the 2nd surgery he said he did so much bowel scraping I can't go to the bathroom without being in severe pain throughout my intestines. Am I thinking clearly on this? Thanks! A
