How Do They Know That a Cyst IS a Cyst?

I had a lap in July and for the past few months I have had a huge cyst develop on my right ovary. I just had a vaginal ultrasound and the doctor didn't seem to think it was a dangerous cyst - I guess he feels it will resolve itself with my next period. How do they know the difference between a regular, functional cyst and something more sinister? When I look on the ultrsound screen I sure don't see how they could tell the difference. I am terrified that it is the start of some type of cancer and it's stressing me out. Also, he said the size of the cyst was 36x42 - is that huge?
