abdominal pain

I am suffering from severe pain in my lower abdomen. The pain resembles menstrual cramps but begins approximately 10 ten days before my period and comes and goes for several days. The episodes of pain last from 5 to 30 minutes and become very intenses before easing up completely and I wake up several times times in the night with severe pain. When the pains start, the episodes come several hours apart but become as frequent as every hour at their worst. My abdomen also feels bloated. The pain usually lets up a couple of days before my period starts and my period is usually pain free. I am 50 years old and have had this problem for several years but it seems to be getting worse. I have tried the heating pad and over the counter pain relievers with little result.
