endo or adhesion?

I have a lengthy history of endo, treated by four laps., continuous BCps, Depo-Provera and Lupron, culminating with a TAH/BSO in 1999 after adenomyosis was suspected. Adenomyosis was verified and endo was excised. I was totally pain free until I tried a small dose of ERT four months after surgery and endo pain returned and resolved within weeks after stopping ERT. I was pain free for nine months and my GYN started low dose ERT again, mainly for newly developed insomnia and the pain returned once again, but this time has not resolved after stopping ERT. I have generalized low aching pain extending at times into my lower back which can increase in severity without precipitating factors and also a tenderness at the left edge of the incisional line but no lumps can be palpated. My Gyn does not believe it is endo, without being on ERT or adhesions, and does not know what course of action to take other than sending me to a general surgeon who is also baffled but thought that probably I had developed adhesions either from the original surgery in 1999 or from prostaglandin irritation from the endo recurrence in Jan. 2000 and Feb. 2001. He does not feel comfortable doing a lap. since I have no bowel symptoms and ONLY have debilitating pain. He stated that a lap done after a hyster is technically very complicated. I feel that an eval. by an Endo Specialist for endo/adhesions is my only recourse but my GYN does not know of anyone in the Mid-Atlantic States for referral. I can not continue to function with the pain level I have and feel that I am in a very complicated clinical situation. Any suggestions on my next clinical actions, because at this point I'm my only medical advocate in searching for a solution to this nightmare?
