Though I would start with a call to your insurance company and straight out ask them for a name you can direct your letter to. This will save a lot of time by your letter not having to go from desk to desk. You are also allowed a second opinion from a general surgeon, so you may want to take that approach with your PCP. State that you saw the surgeon that he/she sent you to and you were uncomfortable with how he handled things. Then explain that you would like to see another one (preferably your choice) for a second opinion. I hope this helps some. Kelli L.
