Breast and Nipple Soreness

I weaned my son about six months ago. One of the reasons that I weaned him when I did was the pain I felt in my nipples. I continue feeling this irritation in both nipples, and my left breast is swollen and painful, as is the upper part of my left arm. Any ideas of what I should do?
